Lease Application Form
This lease application cannot be filled in on-line, you must print it out, preferably on 8½" X 14" (Legal) size paper. Please answer all questions and remember to provide a copy of your most recent pay stub and a copy of your driver's license. Applications can be delivered to Home Team Realty in person or by mail to: HOME TEAM REALTY, P O Box 15217, Oklahoma City, OK 73155. Applications will not be processed without full payment of the Credit Application fee which is $40 for 1 or more people with same last name. Different last names, different reports, fee is $40 each. (Note: Other charges may apply.) Your signature on the application must be notarized if not signed in front of our property manager. App fee must be in the form of cash or money order, make money orders out to Home Team Realty. Call our office before submitting this application. (405) 670-1600
